Advance Care Plan. It is what it says.

Simplify Caring, Your Online Simple Office

The BC Government website says:
“When you write down your wishes and instructions for future health care, you are making an Advance Care Plan. An Advance Care Plan is a written summary of a capable adult’s wishes or instructions to guide a substitute decision maker if that person is asked by a physician or other health care provider to make a health care treatment decision on behalf of the adult.”

Yes, they use long sentences to say simple things. However, this is important. Are you thinking about it? More importantly, have you had this conversation with your chosen representative?

Ask your friends how this conversation is going in their own family? They may be an encouragement to you, or vice versa, you may encourage them to be brave and talk about this with their family.
